Duties Description POSITION DESCRIPTION:

The selected candidate will be a vital team member within the Region 1 Regional Field Office organizational structure. The LMSW will typically provide support to one designated functional team within the Regional Field Office but may provide additional or alternative coverage when Regional Field Office needs deem this necessary. The functional teams within the Regional Field Office include: Front Door, Waiver Services, Eligibility, Self-Direction, Housing, Crisis, Property/Development, Fiscal, and Quality Management/Technical Assistance. The selected candidate will work to ensure excellent quality in Regional Field Office operational standards.

Occasional evening and/or weekend work may be required to meet organizational needs. Occasional travel within the 17-county region may be required.


PRIMARY DUTIES AND RESPONSIBILITIES:

Specific responsibilities will include, but are not limited to:

1. Serve as a Front Door facilitator assisting individuals through the Front Door process.
2. Process Front Door referrals in accordance with the OPWDD Front Door Procedure Manual, ADMs and other guidance.
3. Attend and participate in Front Door related meetings, telephone conference calls and video conferences.
4. Serve as a member of the Regional Field Office providing the full range of social work services to individuals and families.
5. Participate in activities such as screening and/or assessing individuals psychosocial needs, strengths and limitations.
6. Assist with reviewing and assessing individual’s need for OPWDD services and supports.
7. Communicate with key stakeholders, including staff in various departments in the Regional Field Office, Central Office, Care Management Agencies and Voluntary Provider Agencies.
8. Serve as a liaison between the Regional Field Office and the individuals/families we serve, along with provider agencies and other referring entities.
9. Assist with Care Coordination Support Liaison duties as necessary.
10. Assist with Children’s Waiver Liaison duties as necessary.
11. Assist with presentations as necessary.
12. Provide social work services across the various departments in the Regional Field Office as needed.
13. Additional duties may be assigned to assist in supporting Regional Field Office operations.
14. Supervision and training of subordinate staff may also be required.

Minimum Qualifications Open Competitive: license and current registration as a Licensed Master Social Worker in New York State.

Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

• If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.

• The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
